Project Initiation Form
If you would like to have a project completed using departmental end of year funds, please use the link provided below to submit a project initiation form to Facilities Services. After opening the link, simply click “Add Document” and complete all sections of the form, then click “Submit“.
After submitting a request, a Facilities project manager will be assigned and will have an estimate completed by March 31, 2017.
Once the estimate is completed, the form will be routed electronically back to the appropriate departmental budget approver. If approved, the form will be routed electronically to the appropriate vice president for final approval. Once approved, a project manager in facilities services will contact you to schedule a kickoff meeting.
Please keep in mind that projects greater than $100,000 are subject to competitive bidding and should allow additional time for the project to be completed.
